Scheduling, Referrals and Appointments
- No Appointment Needed!
If your procedure does not require special preparation, just bring or fax the signed order to the most convenient location and we will process the request and perform the imaging
- Scheduled Appointments
Step One: Your doctor faxes us the order for your imaging procedure and we will contact you to schedule the appointment
Step Two: Your doctor’s office will contact us to schedule your appointment, usually while you are still in the office, and will give you the written imaging order to bring to your appointment. We will remind you at this time of any special preparation your examination will require!
- What you need to bring to your appointment
1. Your physician signed order for the imaging procedure
2. Your current insurance card (we will file for you)
3. A list of current medications if required by the procedure preparation sheet
- Exceptions for written orders (because there is always and exception!)
1. Screening Mammography – Our sites are approved by the State of Ohio to accept women, age 35 and older, who self refer for mammography
